A computed tomography (CT) is useful in diagnosing and determining the extent of BSPDC. Here we report a 48-year-old woman presenting to the emergency department with a sudden onset conscious loss followed by general convulsions. She had undergone a total thyroidectomy for a benign thyroid goitre 20 years ago. A marked decrease in the serum calcium level (4.7 mg/dL) was noted. The diagnoses of hypoparathyroidism and primary hypothyroidism were also confirmed thereafter. A non-contrast head CT (figure 1) revealed symmetric intracranial calcifications, including subcortical white matter, thalami, basal ganglia, caudate nuclei and cerebellums. After the correction of hypocalcaemia with intravenous

Striopallidodentate Calcification and Progressive Supranuclear Palsy-Like Phenotype in a Patient with Idiopathic Hypoparathyroidism
We present a 77-year-old woman with levodopa-nonresponsive parkinsonism, dementia, and supranuclear gaze palsy on vertical and horizontal gaze. Laboratory findings were consistent with idiopathic hypoparathyroidism, and brain computed tomography showed extensive bilateral calcifications of the basal ganglia, centrum semiovale, dentate nuclei, and cerebellar white matter. These results illustrat...
متن کاملTranscranial sonography of basal ganglia calcifications in Fahr disease.
Fahr disease, also known as bilateral striopallidodentate calcinosis, is a rare degenerative neurologic disorder characterized by almost symmetric calcifications of the basal ganglia, the cerebellum dentate nuclei, the thalami, and the white matter of the cerebral hemispheres. Bilateral striopallidodentate calcinosis manifests as familial autosomal dominant but also as sporadic forms. Movement ...
متن کاملA case of paroxysmal kinesigenic dyskinesia in idiopathic bilateral striopallidodentate calcinosis
Paroxysmal kinesigenic dyskinesia (PKD) is presented as a short paroxysmal attack of focal or generalized involuntary movement. Bilateral striopallidodentate calcinosis (BSPDC) is referred to as Fahr's disease and is characterized by the calcification of the basal ganglia, cerebellar nuclei and thalamus. Most common presentation of BSPDC was a parkinsonism, and PKD has also been reported to few...
متن کاملExtensive Bilateral Intracranial Calcifications: A Case of Iatrogenic Hypoparathyroidism
This is a case of a 69-year-old male patient with long-standing iatrogenic hypoparathyroidism after total thyroidectomy. The clinical evaluation revealed mild neurological symptoms and excessive brain calcinosis. Intracranial calcification that affects structures other than the basal ganglia and the cerebellum is a rare manifestation of postoperative hypoparathyroidism. Detection of brain calci...
متن کاملIdiopathic basal ganglia calcification as a differential diagnosis of parkinsonism: a case report in an older adult.
We present the case of a 76-year-old man who was admitted to the emergency room of Dario Fernandez General Hospital, ISSSTE (Mexico City, Mexico) with slurred speech of a few hours of onset. His relevant background included hypertension since he was 68 years-of-age without current treatment, and a posterior cervical laminectomy because of medullar injury when he was 72 years-of-age. During the ...
